Shingles Pain – Some Flippy Ideas

Here’s a thought about shingles pain relief. I didn’t think of this. Neuroscientists have been studying it for some time. However, in my case, my focus is on PHN or Postherpetic Neuralgia. PHN is pain that doesn’t go away after the shingles has subsided. About 12% to 15% of patients develop PHN after herpes zoster. More than 1 million people (mostly over 60) receive a PHN each year. For some people, the pain is severe, sometimes lasting for months or more than a year.

Here’s the idea. I recently read an article by Atul Gawande in The New Yorker about postherpetic itch. His article is long, but if I can get the gist of it together, he’s reporting on changing perceptions of perception and pain. In this newer view, perception is more of a structure in the brain than simple impulse reception. Some research seems to suggest that about 80% of what we see and otherwise perceive are mind maps rather than direct impulses from the world.

A good example is the phantom pain of an amputee losing a limb. Amputee experience complex hallucinations from limbs that do not exist. They can feel as though water is dripping from the missing limb, that it is hot, cold, or in some strange position. Apparently, there was no feeling in the missing limb, and cutting the stump short did not alleviate it. So the problem is not the damaged and frayed nerves at the end of the stump. It comes from the brain itself.

Into the frame. It has been found that using a mirror box with the amputee’s functional arm and its reflection can “fool” the patient into thinking he has two limbs again and miraculously stop the hallucination. The brain structures responsible for the sensation of the missing limb are reset. In fact, some amputees report that the phantom limb retracts back into the stump and stays there.

So how can a person with PHN see his body using mirrors at right angles to each other? Set up the mirror box to replicate the healthy, pain-free side into the mirror box to make it look like a full body. He has to stay still to be effective. Conveniently, shingles only occurs on one side of the body or the other. This may trick the brain to reconsider its guessing game about reality and stop phantom or neuropathic pain.

Like I said, I didn’t come up with this line of thought, but applying it to PHN might be a new thing to watch. Treatment of shingles pain, especially PHN, is very challenging. What if something as simple as a mirror box could bring relief? See why I’m calling it a flippant idea.
