Fever: Treatment or Not?
Fever is one of the scariest problems parents face in their children, but it doesn’t have to be. Fever is one of the body’s defense mechanisms in fighting infection, and is not harmful in itself. The normal body temperature is about 37 degrees Celsius, or 98.6 degrees Fahrenheit. This varies somewhat from person to person, and tends to be higher in the afternoon and evening than in the morning as part of what is called diurnal variation.
With infections the body responds by raising its body temperature. A fever is defined as a temperature above 38 degrees Celsius, or 101.4 degrees Fahrenheit. Fever is one of the classic or cardinal signs of infection, and can actually help the body fight and kill bacterial and viral pathogens. A fever does not harm the person with the fever. A fever can naturally raise the body temperature to slightly above 105 degrees, but is self-limiting in how high the temperature can rise.
Heatstroke is a particular problem, caused by a combination of dehydration and high ambient temperature. In this situation the body temperature rises due to failure of the body’s self-regulatory systems, and the skin becomes dry and hot, and confusion, shock and death may follow. This basically never happens with a fever of illness.
Treatment of fever should be focused on making the febrile patient comfortable, and not on achieving normal body temperature. There is no evidence that treating fever shortens illness, prevents complications, or is important for reasons other than comfort. Tylenol and Ibuprofen are the medications most commonly used to treat fever, although in adults aspirin is also an effective option.
